NURSING SERVICES

The Private Home Care nurse utilizes nursing skills as a first line direct patient care provider and as an integral member of the interdisciplinary treatment team. The nurses works under the direction of the Medical Director. Ensure quality and safe delivery of home care services.

  • Participates in development and implementation of client plans

  • Participates in client case conferences according to home care agency policy and procedure, if requested.

  • Information regarding client plans of care will be submitted to the Home Care SUPERVISOR in a timely manner. Implements current Certified Nurse Aide services.

  • Service plans of care are discussed with the Supervisor on regular basis.

  • Client assignments and reports are received from the Supervisor.

COMPANION SITTER

Provides continuous companionship for the client while on duty.

  • Assists in meal planning, purchases of foods, and food preparation.

  • Washes dishes and cleans the kitchen after meals.

  • Assists the client to live in a clean, healthy, and safe environment.

  • Observes the client’s general physical, emotional and mental conditions, and reports all changes in condition to Registered Nurse (RN).

  • Documents and keeps appropriate records of all service activities while in the client’s home.

  • Provides medication reminders and supervises the self-administration of the medication(s).

  • Maintains strict confidentiality of client, family, and caregiver information.

Personal care services Georgia, Mississippi

PERSONAL CARE

Provides continuous companionship for the client while on duty.

  • Assist clients with personal hygiene, including shower, tub or bed baths, oral care, hair and skin care.

  • Assist clients in the use of toilet facilities, including bed pans.

  • Assist clients in and out of bed, excluding the use of mechanical lifting equipment unless trained and documented as competent.

  • Assist clients with walking, including the use of walkers and wheelchairs, when applicable.

  • Assist clients with self-administration of medications. Meal preparation and feeding, when required.

  • Assist with prescribed exercises when the client and the aide have been instructed by the appropriate health professional.

  • Record and report changes in the client’s physical condition, behavior or appearance to supervisor or Case Coordinator.

  • Documenting services delivered in accordance with New Progressions, LLC Private Home Care policies and procedures.

  • Assists in meal planning, purchases of foods, and food preparation.
