Community Mental Health Centered Services
Programs and services are designed both to benefit and meet the needs of the adult, child and their family in order to ensure increased attainment of necessary life skills, and continued preservation of the family unit. Our CORE Outpatient Services include the following:
Child/ Adolescent Mental Health
Our targeted, short-term programs support children and youth with serious emotional disturbances who need more intensive care. These services improve academic and behavioral outcomes while promoting community living, preventing hospitalizations, and minimizing school disruptions.
Therapeutic interventions focus on positive feedback, self-esteem, and social skills, with additional training in impulse control, anger management, problem-solving, and conflict resolution, tailored to individual needs.
Psychiatric/Physician Services
This service is for both children and adults. This service is more of a medical nature provided by medically trained personnel to address medical conditions related to the person’s mental illness or emotional disturbance. Medical services include medication evaluation and monitoring, nurse assessment, and medication injection.
Outpatient Therapy
Outpatient Therapy is available for both adults and children. This service includes an initial assessment along with individual, family, group, or multi-family therapies. It involves face-to-face interactions, such as conversations or play therapy, conducted by licensed professionals or credentialed therapists. The goal is to establish a therapeutic relationship to address and resolve symptoms of mental illness or emotional disturbances.
Community Support
This service, available for children and adults, is delivered by certified DMH/C and DMH/P providers through mobile Community Support Specialists. Services are tailored to individual needs, varying in hours, type, and intensity. The goal is to provide measurable, personalized support to help individuals succeed in the community, access necessary services, and improve participation in school, work, family, and community life.
Targeted Case Management
​This service is for both children and adults as well. Targeted Case Management Services are services that provide information/referral and resource coordination for a person and/or their family, or other supports. Targeted Case Management Services are directed towards helping the person maintain the highest possible level of independence. Case managers monitor the individual service plan and ensure team members complete tasks that are assigned to them, that follow-up and follow-through occur, and help identify when the person’s team may need to review the service plan for updates if the established plan is not working.
Psychosocial Rehabilitation (PSR)
Adult Only
The purpose of the service is to promote recovery, resiliency, and community integration by maintaining the person’s optimal level of functioning and preventing psychiatric decompensation, thereby decreasing the risk of unnecessary hospitalizations and the need for higher level intensity services. New Progressions, LLC utilizes the Club House model for this service. Service activities aim to alleviate current symptomatology and address the person’s underlying condition by reducing the negative effects of social isolation, promoting illness education, creating and monitoring wellness action plans, and the development of other coping and independent living skills.
Peer Support
Peer Support Services, available for children and adults, are delivered by Certified Peer Support Specialist Professionals (CPSSPs) who draw on their own recovery experiences to assist others with mental health or substance use challenges. CPSSPs provide mentoring, guidance, and non-clinical support using skills gained through training from DMH. By fostering shared understanding and empowerment, they help individuals engage in recovery and reduce relapse risk. These services extend treatment beyond clinical settings into daily life, complementing clinical care within the behavioral health system.
Child Day Treatment
Children Only
Day Treatment Services are targeted, short-term programs for children whose less intensive interventions have been unsuccessful. These mid-level intensity services focus on academic and behavioral outcomes, promoting community living and well-being for youth with serious emotional disturbances. They provide an alternative to more restrictive options like hospitalization or residential treatment, aiming to prevent psychiatric admissions and minimize school disruptions. Therapeutic activities address underlying conditions and symptoms through behavior management, positive reinforcement, social skills training, and other needs-based components such as impulse control, anger management, and problem-solving skills.
